Elderly Services

Amy LaChioma, Elderly Services Coordinator, provides case management to Monroe residents age 55 and older. She serves as a single point of entry for seniors, their family members and/or caregivers on a variety of programs, services or support. She will assess client needs and match them with appropriate programs or services. She can assist with paper and online applications.

Caregiver Information

Serves as a single point of entry for seniors, their family members or caregivers on a variety of programs, services or support. Will assess client needs and match them with appropriate services, including but not limited to: energy assistance, S.N.A.P., Farmer Market Coupons, Connecticut Homecare Program for the Elderly, Veteran’s Aid and Attendance Benefit, Respite Care and more.  

Chore Service

As part of a SWCAA grant, we can help Monroe seniors who have difficulty, financially or physically, completing any of the chore services listed: heavy housework, yard work, minor home repairs, etc.  Your request will be reviewed and if it meets the criteria, a form 5 will be completed and the senior will be provided with a list of vendors who signed on for this program.    

Outreach Program

Visitation of home bound seniors for welfare checks, conversation, assist with applications and more. Provide outreach and education to senior groups in the community and at the center.

Energy Assistance

Completes applications or makes referrals for various local, State and Federal programs that assist with Energy needs with agencies such as Alliance, Operation Fuel and Eversource. Programs are based on income and assets.

Medicare

Assists those new to Medicare or turning 65 on the insurance options with Medicare. During open enrollment will review Medicare Advantage plans, Prescription Drug plans and Medicare Supplement Plans.

Medicare Assistance Program

A program that helps qualifying seniors with their Part B premiums and for some other benefits may apply. Program is based on the seniors’ income.

Food Programs

For those eligible makes referrals to our Monroe Food Pantry; coordinates the Farmer Market Coupons and Veteran’s gift certificates; assists applicants to complete SNAP applications or redeterminations.
